Arrowhead Hospital (AH) currently provides the following benefits to employees who have met the eligibility requirements. The benefits are based on the number of scheduled hours worked per week. Your benefits go into effect the first day of the month following your hire date, in a benefit eligible position. The company shares the cost of your medical, dental and vision coverage. |
LIFE INSURANCE Arrowhead Hospital rounds term life insurance equal to one times your annual salary for life insurance. 1 times your annual salary is paid by Arrowhead Hospital. You may elect up to three times your annual base pay to a maximum of $500,000 in term life insurance, which includes matching AD&D. Optional dependent life is available and you may purchase a policy for your spouse up to ½ of the amount that you carry for life insurance and up to $10,000 for your eligible children. | ||||||||||||||||
GROUP MEDICAL INSURANCE The medical plans currently available are HMO and POS+. Both plans include coverage for medical care, prescription drugs and behavioral health. The amount paid varies by the type of coverage the employee elects, i.e., single, or if you elect to have dependent coverage. |

| DENTAL INSURANCE We offer two dental plans. The DMO is a managed plan that you must go to your primary dentist for all dental care. The PPO is a plan that you may go to any dentist you prefer. The PPO has a $1,500 maximum per calendar year for each enrolled person. Both offer 100% paid on diagnostic & preventative dental care without deductibles. | LONG-TERM DISABILITY Employee pays the full cost of coverage based on your age at the time of eligibility or beginning of the plan year, which is January 1. You may purchase a policy with a 90 or 180 day waiting period. EARNED TIME OFF (ETO) Full time and part time employees earn ETO each pay period starting on your first day of employment.. You are eligible to take ETO after completing 90 days as a full time or part time employee. Full time non-exempt employees and exempt non-managerial employees in positions regularly scheduled for 40 hours per week earn ETO as follows:

| EXTENDED ILLNESS DAY (EID) Full time and part time employees earn EID each pay period starting on your first day of employment. You are eligible to take EID after completing 90 days as a full time or part time employee. Employees may take EID after being out ill for 4 continuous days. If you are having inpatient or out patient surgery or are injured at work and out due to a worker’s compensation injury you can use your EID right away. Full time employees are awarded EID at the rate of 1.85 hours each pay period (6 days per year). | EMPLOYEE ASSISTANCE PROGRAM (EAP) Arrowhead Hospital offers an EAP program through CONTACT.
